NOAH created the Population Health Team – usually referred to as Pop Health – in February 2021. This team was established to provide greater support to our patients and give them tools including education, self-management skills, and resources they need in order to improve their health and wellness.
The Pop Health team now has 28 members working in several different areas:
- Nurse Triage
- RNs available to answer patient questions and triage healthcare needs before seeing a provider.
- Monday – Friday from 7 a.m. to 7 p.m.
- Saturday from 7 a.m. to 3:30 p.m.
- QuickCare
- Virtual healthcare with a provider for acute needs.
- Outreach Team
- Team of MAs who connect with patients to close gaps by scheduling services including well-child exams, colorectal screening, and diabetic follow-up appts.
- Nurse Navigators
- RNs at clinics providing education and care coordination to patients in need of assistance with managing chronic conditions
