NOAH’s Commitment to Value-Based Care

Last year, NOAH started working directly with AHCCCS (Medicaid) to get more support and revenue. Because of the high-quality care we provide, we received extra income through these contracts. This additional revenue allows us to expand our care teams to better serve our patients. AHCCCS is especially concerned about well-child/preventive exams for those 21 and younger. Sadly, in Arizona, there have been low rates of children completing these visits in recent years.

Throughout the year, the NOAH Population Health team, in collaboration with the PEC and Community Resources teams, have been diligently reaching out to and scheduling well-child visits for children and young adults associated with NOAH, both returning patients and new patients. Utilizing phone calls, text messages, and email campaigns, they have engaged with families, emphasizing the importance of well-child visits. Additionally, efforts have been made to contact those who have previously missed or canceled appointments, addressing any barriers that may hinder a child from receiving necessary care. The goal is to significantly improve well-child visit completion rates by the end of 2023.

  1. Well-child exams
  2. Breast cancer screenings
  3. Cervical cancer screenings
  4. Diabetic control (HbA1c < 9%)
