Rent and Utility Assistance

Through an additional partnership with the Department of Economic Services (DES), NOAH’s Community Resources team is now able to assist patients and community members with the application process for Rent and Utility Assistance.

What does this mean?

NOAH’s Community Resources team can link qualifying patients and community members with these services and directly submit/upload documentation to the DES portal on their behalf. 

Can NOAH employees apply for this assistance?

Yes, however, due to conflict of interest and contract limitations, NOAH employees will be routed to DES or other partner agencies that can assist.

What are the requirements to qualify for these benefits?

Please refer to the Rental Assistance Document Checklist for eligibility requirements.

How do we connect patients or community members to this assistance?

The process remains the same with standard referrals to the Community Resource team or a telephone encounter routed to our pool. Whenever possible, please include a diagnosis code and note regarding the specific need as this helps when conducting outreach.
