What is Health Related Social Needs (HRSN)?

Health-Related Social Needs (HRSN), previously called Social Determinants of Health (SDOH), are basic social and economic needs that affect a person’s health. Things like not having enough money, difficulty accessing healthy food, safe housing, transportation, or healthcare that put individuals at risk for worse health outcomes and increased health care use. 

Why is this Important?

At NOAH, we focus on treating the whole person. That means making sure our patients have what they need—like food and transportation—because without these, other health issues can’t improve. By screening for these needs, we can better prioritize care and resources for our patients.

How Will We Screen for HRSN?

Beginning November 1, we will use the PRAPARE screening available in Epic. Please reach out to your supervisor for specific workflow guidelines.

Who Will Get Screened for HRSN?

All new patients throughout November and December 2024. After the new year, all patients will be screened at least once a year or as clinically necessary.
