Patients with the following health plans can schedule their own transportation by calling the associated numbers below (or a NOAH team member can schedule on their behalf via the Veyo portal):
- UnitedHealth Care Community Plan 1-888-700-6822
- Care 1st: 602-778-1800 or 1-866-560-4042
- Health Choice: 602-386-3447 or 1-800-322-8670
Criteria
We understand that determining the necessity of transportation can be complex. We assess the urgency of transportation based on individual patient needs. For instance, if a patient is uninsured and scheduled for a preventive appointment but experiences transportation issues, we consider factors such as running out of medications, needing test results, or other urgent healthcare needs. In such cases, transportation may be provided.
Patients using NOAH accounts should note that transportation is only provided to NOAH facilities, SMIL, and, soon, the NOAH Pharmacy when needed.
Mileage and Destination
If patients are using an AHCCCS health plan, there may be mileage stipulations they should discuss with their health plan provider. For NOAH account pay (uninsured/underinsured or on SFS), trips exceeding $50 will undergo review. We aim to schedule transportation to the nearest NOAH clinic, but we also explore virtual and remote options when available.
Contacting Transportation Services
Patients with eligible plans can call the provided numbers to schedule transportation directly. For those who do not have one of these plans, calling Community Resources for assistance is recommended. Patients should call 480-882-4545, choose Community Resources and then request transportation services.
Request Timing
To ensure timely transportation, requests should be made at least one week in advance when using NOAH account pay. For AHCCCS health plans, a minimum of three days’ advance notice is required.
Additional Information
Transportation services cover support people or family members, but we aim to limit it to one support person or child, as larger vehicles can increase transportation costs. Patients are responsible for bringing their child’s car seat when needed.
How to Submit a Request
Any employee who needs access to the Veyo Portal must complete the required training. Typically, front office staff and some PEC (Patient Engagement Center) staff should have access to the portal. Transportation requests will either be approved instantly or placed in the approval queue, depending on the specific criteria.
