MyChart access and proxy guidelines pertaining to the age of the patient have changed as of Sunday, February 19, 2023. Proxy access for a child will now apply to children 12 and under (previously 11 and under) and adolescent will now apply to adolescents age 13-17 (previously 12-17). This change has also prompted updates to the MyChart proxy access forms for adults and children.
New Forms
Please ensure old versions of the form are discarded and the new forms dated February 2023 are being used going forward.
