DES will no longer accept rental assistance applications effective Sept. 1, 2024 due to the depletion of funds. NOAH’s Community Resources team will continue processing rental assistance applications through Aug. 30, 2024.
NOAH will also continue assisting with LIHEAP (Low Income Energy Assistance Program) energy/utility applications until further notice. Although resources are limited, we can still connect individuals with local agencies that may be able to assist with housing resources. If you have a patient in need of housing or utility assistance, please send a referral to community resources.
Referral tips:
- Urgent/emergency status should only be used for truly time-sensitive situations, which our team will prioritize for same-day handling.
- The diagnosis code can be any Z code related to the patient’s social needs (more details are on the tip sheet).
- Please ensure you have the patient’s consent before referring them to the Community Resources team, as it facilitates smoother communication when we make initial contact.
