NOAH Community Resources is excited to announce the addition of a Community Health Worker position to our team. Please join us in welcoming Elizabeth Suarez who will fill this important role. The Community Health Worker will partner with patients and providers to:
- bridge any gaps in patient/provider communication
- motivate patients to take charge of their health
- develop self-management skills for patients with chronic disease(s)
- create patient accountability for scheduling and attending appointments
- follow-up and provide ongoing support as needed
To maximize the impact of this role, initial efforts will specifically focus on Desert Mission patients who are diabetic, had a blood glucose level of 9.0 or higher on their last blood test, and haven’t been seen in a year or more. The goal is to improve patients’ A1C to under 9.0 in under 9 months. #9under9
The Community Health Worker will not be taking referrals at this time. As we identify the program’s challenges and best practices, we will evaluate needs for expanding the program to other NOAH health centers and/or to address additional illnesses.
