Registration is now open for our special employee health event sponsored by our benefits plan administrator, Ameriben. This free event will feature health screenings, vaccines, coffee, and benefits swag! Not to mention some cool prizes for participation!

Health Screenings

Employees will have the opportunity for a 15-minute confidential health screening completed by an external party that will include a finger stick and insights on your total cholesterol, HDL, LDL, triglycerides, glucose, blood pressure, height, weight, BMI, waist circumference, and metabolic syndrome analysis. All free to you!

Registering for a time is preferred, but if you register, you can come any time during the event when patient care allows. 

To schedule your appointment:

1. Click here. 
2. Under the NOT YET REGISTERED? box, enter the Screening key: MMA41 and click Submit. 
3. Enter your assigned default username and password and click Verify My Account. 
   • Username: First Initial + Last Initial + DOB (MMDDYYYY) 
   • Password: Last Name + Last 4 SSN 
4. Follow the prompts to schedule! 

Vaccines

Employee Health/Quality will be onsite to provide flu and COVID vaccines. The 2024-2025 flu vaccine will still be required for all employees. Declination will require masking during flu season. COVID vaccines will be optional for 2024-2025.  If you complete your flu requirement prior to or during the event, you’ll be able to grab a free coffee from Blue House Coffee who will be onsite for the event!

Benefits Information

Open Enrollment for the 2025 plan year will run October 30 to November 13. We are still working on finalizing all the renewals for the new plan year. We hope to be able to provide initial information about our renewal and be available to answer any questions you may have about benefits during this event. We’ll also have some cool swag from our vendors.
