Your New ID Card
- This card is needed for your medical, prescription, and Telehealth insurance coverage as well as the employee assistance plan.
- New identification cards for 2022 were mailed to your home address in late December.
- If you are a new hire, you will receive your identification cards in the mail at your home address about two weeks after enrollment.
- This card includes a new ID number that you will need to give to your health care provider (even if they have your old ID number on file).
- You will be issued two cards that include your information as well as your dependent information.
- If you have not received your new insurance cards in the mail please call AmeriBen directly at 855-961-5370.
Online Account Access
- If you previously registered with myameriben.com you will need to create a new account using your new ID number to access medical claim information for 2022.
- Information for 2021 claims (or earlier) can be accessed with your previous account (HonorHealth login).
- You can also access an electronic copy of your ID card and send it straight to your providers via the MyAmeriben website.
Delta Dental of Arizona
Delta Dental of AZ will issue two ID cards to all enrollees. If you have not received your new insurance cards, please reach out to Delta Dental of AZ directly at 800-352-6132 or via email. You can also access your benefit information 24/7 by registering your account at deltadentalaz.com.
VSP Vision
VSP Vision does not issue ID cards. If you have enrolled in vision coverage, you can register your account at vsp.com and print a generic ID card. The process without an ID card is simply letting the provider know that you are a VSP member and providing your name and/or the last four digits of your social security number. A dependent would follow the same process as the employee and provide the name of the employee and/or the last four digits of the employee social security number.
Your provider will use this information to request authorization for services and then bill the claim to VSP.
HealthEquity HSA
New debit cards will NOT be issued for employees who enrolled in the HeathEquity HSA program as HonorHealth employees. Your account has automatically rolled over to NOAH’s HSA for 2022.
New to HSA? HealthEquity will issue your debit card about two weeks after you complete the required verification process.
WEX Health FSA
If you elected to participate in the FSA, WEX Health will mail your new FSA debit card(s) to your home address about three weeks after enrollment.
