What is Unite Us?
A technology platform that will be interfaced with EPIC where organizations can place Social Determinants of Health (SDoH) referrals to Community Based Organizations.
Here are some of the features:
- Closed-loop referrals with outcome tracking
- No wrong door — easily connect clients to resources in one platform
- Screenings and assessments for identifying client needs
- Alerts and communications about client case progress
- Data dashboards, analytics and outcomes
- Resource directory
How might this impact me?
You may see an icon that may be new to you, please disregard as this is only for Community Resource staff, Population Health staff, and or Behavioral Health staff:
The platform will be used by the Community Resource team, Population Health, and in the future the Behavioral Health team, to place referral to Community Based Organizations.
What if a patient has an SDoH need?
For patients experiencing a need in SDoH, please continue sending the referrals to our Community Resource team.
