To complete your renewal application, please log into your AZ Department of Health Services portal.
Working as a NOAH care provider is rewarding in many ways including the opportunity to apply for Loan Repayment assistance through the Arizona State Loan Repayment Program.
As a Federally Qualified Health Center, NOAH qualifies as an eligible service site and many of our providers have been awarded funds through this program.
Eligible Providers
- U.S. Citizen
- Physician in at the field of:
- Family Medicine
- Internal Medicine
- OB/GYN
- Pediatrics
- Psychiatry
- Geriatrics
- Dentist
- Advance Practice Provider: Physician Assistant, Nurse Practitioner
- Pharmacist
- Behavioral/Mental Health Provider: Clinical Psychologist, Clinical Social Worker, Professional Counselor, Marriage and Family Therapist
Eligible Service Sites
- Public or private non-profit facilities in a federally designated HPSA or rural private practice sites in a HPSA or AzMUA
- Accept Medicaid, Medicare, and qualifying health plan of the Health Insurance Marketplace assignment
- Implement a Sliding Fee Scale
