If you currently have dependents enrolled in NOAH medical plans, you will be receiving a letter in the mail soon from the administrator of our medical plan, AmeriBen, regarding a routine audit.
The letter will request that you submit proof of dependency for each dependent on the plan within 30 days of the date of the letter.
Proof of dependency may include a copy of the following:
- Birth Certificate
- Marriage License
- Domestic Partner Affidavit
This information will be processed and held within all HIPPA privacy practice requirements.
If proof of dependency is not received within the allotted timeframe, your dependents will be removed from the plan and any previously paid claims may be investigated as potentially fraudulent.
Going forward, proof of dependency will be required for any new dependents at the time of enrollment. This information may also be requested in the event of another dependent audit.
