Community Resources Department is Here to Help

Our Community Resources team is a critical part of NOAH’s whole-person care. The team has grown significantly during the past year so let’s understand the support available through this NOAH team.

Only 20% of a person’s overall health is determined by access to and quality of healthcare. The remaining 80% is made up of other factors, known as Social Determinants of Health or SDoH. SDoH includes things like housing, workplace, diet, exercise, access to healthy food, education, employment, income, and more.

Our Community Resources team is well-versed in the many programs and organizations that can address specific SDoH factors for patients, non-patients, and staff, including:

  • Employment
  • Financial Assistance
  • Access to Food Banks
  • Health Insurance Coverage
  • Sliding Fee Discount Program
  • Education
  • Homelessness
  • Domestic Violence
  • Drug or Alcohol Use/Abuse
  • Nutritional Guidance
  • Behavioral Health Services

Addressing SDoH as part of routine healthcare can improve patients’ overall health. NOAH’s approach to whole-person care gives us the opportunity to further support people by referring them to our Community Resources team.

How You Can Help

When you meet people who may be struggling, ask if you can refer them to our Community Resource team. There’s no cost, no commitment, just a quick conversation to determine if there’s something we can do to help. Referrals can be submitted via Epic. Community Resources typically follows-up with referrals by phone within a few days.
